They are enabled in the preferences, but they are not appearing systemwide.
As a test, I tried setting them to different types of smart quotes for different languages in the preferences.
It still doesn’t work. Any ideas?

Could you be specific about where they do not appear?
3rd party apps like Word have their own system for this.
Apple apps may also need to have it turned on in Edit > Substitutions

Option key + quote usually yields smart quotes.
Some programs offer smart quote in their preferences for automatic conversion.
Keep in mind, when copying smart quote to plain text services like some email software, the recipient may not see it as a quote at all, because smart quotes are not a consistent ASCII code. It will only be consistent for people using the same code rendering engine.
Of course, but it would be way better to have it happen automatically.
