CarPlay is 100% run on your CarPlay-compatible iPhone.  The screen in your car displays what your iPhone sends to it.  Therefore, any updates to CarPlay occur when you update iOS on your iPhone.

With that said, car manufacturers can also update their "Infotainment" systems, and those updates could include CarPlay compatibility fixes.  Each car manufacturer is different, so you'd need to check with them to see if an update to your Infotainment system is available.

And, of course, the CarPlay-compatible apps you use can be updated at any time by the app developer.

As said above, you update CarPlay when you update the OS.
