My neighbor is 89 and legally blind and hard of hearing. She has an iPhone (4-6?) which she just uses for calls. Her challenges are:
Is there any way to lock the volume at the max level, answer a call w/o needing to swipe the screen, and default to speakerphone for all calls?

As lobsterghost1 points out, this antiquated iPhone may be too obsolete to have these features.
We all have to say Hey Siri to get Siri to respond. Until iPhone 6s, iPhone became capable of responding to Hey Siri, without having to touch a home or power button first. Test your neighbors phone, with it sitting on a counter, with the screen not on and say something like "Hey Siri, what time is it?" If Siri responds, without the phone needing to be touched when a phone call comes in, your neighbor can simply say "Hey Siri, Answer the Phone."
Your neighbor could use Hey Siri to answer a phone call, but unfortunately, the first phone which permitted the use of Hey Siri was iPhone 6s and since your neighbor has an antique, no longer supported iPhone, that won't work for her. Otherwise, I can't think of a way to help your neighbor as their iPhone is simply too old to use newer features available on newer phones.
