There is no default volume setting. You adjust the volume with the buttons on the side of the phone, and it will remain where you set it.

The volume buttons are context sensitive. That is, they only affect the volume for what you are doing. When the Home screen is displayed they adjust the ringer and haptics volume. On a phone call they adjust the listening volume. In an app such as a game the adjust the game sounds. When setting an alarm they adjust the alarm sound. For Siri they adjust Siri’s spoken level.

There is one automatic setting, called Attention Aware. This is on by default, but you can turn it off. It will lower the volume of the ringer or alarm when you look at the phone or pick it up.

Increasing Default Volume on an iPhone:

Default App Volumes: Apps have their own volume settings, some of which may have default volume settings, though the iOS overrules it all. So, if the iPhone is muted in full, then you're just not going to hear the difference from the app.
