I figured it out. It's unfortunate that Apple is not acknowledging this or doing anything about it. They replaced my iPhone and shortly thereafter, found a solution.
In the control center when on a call make sure "mic" mode is set to "standard" and not "isolation". After I switched to standard, never had the problem again. I was ready to get an Android if it couldn't be solved. Apple was trying to blame it on the cell phone carrier. Let me know if it works for you, good luck!

I fixed mine by turning off conversation boost in control center w AirPods hooked up, also noise cancellation, then it all worked as it should, turned on noise cancellation again and conversation boost and all is working fine, just toggling them worked. I have kept conversation boost off though just to be sure.. but multiple phone calls are all crystal clear and no mic issues.
I finally solved my issue. Turned off “mic isolation” in the control center and set it to “standard@. Problem solved for me after over a year of dealing with carrier and Apple. It’s the phone everyone no matter what Apple tells you, not the carrier.
So much for Genius (NOT) Bar at Apple Store and the numerous senior advisors I spoke to over the last year that could not solve the issue.
I discovered that the volume control for speaker phone mode is independent of the regular iOS volume. This seems to be part of the 17.x update.
Make a call, set to speaker, and then adjust the volume up and it will all work fine.
This was driving me nuts as speakers worked all other apps except in the phone app.
If you have a hearing or sound issue on your device then please try some of these processes to set it right
If the above options do not mitigate the issues then Contact your carrier and then Contact Apple Support

My iPhone 13 Pro Max had the same issue when on phone calls through Apple AirPod Pro, speaker, or iPhone. Turning iPhone off and on again did not help. I found that when I muted then unmuted the phone while on a call, the problem went away. I am not certain why that worked, but it failed to work on 10 calls over 2 days and has worked on all 4 calls since the mute/unmute. Good luck to all, as this was an extremely frustrating bug.
Just to be clear selecting "mic mode standard" on speakerphone in control center does not work for you? If not, definitely go with diagnostic mode route.
I did not have any issues with Factime, only speaker phone. My problem was with "mic isolation" setting on speakerphone causing the issue. Moved to "mic mode standard" and voila, problem gone. I don't have a clue how to go into diagnostic mode and if you don't make appt. at store for them to do it. I'm so sorry you're going through this, I know exactly how you feel, I was there.
I fixed it, same problem cannot hear or talk phone call but can via whatsapp microphone just doing fine.
Here how to fix
settings -> phone -> wifi calling turn on
hope help someone
I solved my issue finally. Genius’ the are NOT at the store or Apple support. I disabled “mic isolation” in the control center and set it to “standard”. It’s a miracle! Let me know if it works for you, hope so!
Try disabling “mic isolation” in control center and set to “standard”. That worked for me. Hope it works for you!
I fixed mine! If you are using any case other than an Apple case the case interferes somehow. I purchased a clear Apple case (bottom is cut out from speakers and charging port) and it’s working normally now. I can use speaker and people can hear me.

Yes. The speaker not working was corrected for me on iPhone 14 by updating from 17.03 to 17.1.
I had the issue with the phone speaker. Reflashed it from dfu mode etc. brought it to apple store they have this little doc that does an audio test of sweeping tones on all the speakers. That fixed it. Guy tried to tell me the speaker was dirty…. Riiiiight
