As long as you have the phone plugged in while you're using the Hotspot feature, it will not affect your battery's overall lifespan.

If you do not have it plugged in, yes, in theory, it will shorten the battery's lifespan because you will need to charge it more often, thus use up charge cycles more quickly.

No it does not affect the battery lifespan. Even i have been using it but its affect the battery health when you charge the iphone more than 2 times daily due to battery drain using hotspot.
