Are use my cash app card to purchase apps from the App Store. I had those apps refunded because I ended up not needing them. Can I get that refund sent to my Apple Pay instead of my cash app account?

Apple Pay holds not funds, its just a payment conduit not an account. Do you mean a specific credit or debit card added to the Wallet? Your Apple Card? something else?
With that said, refunds go back to the payment method used for the original purchase being refunded.
If you used Apple Cash, it will be refunded to that, and can then be transferred to a bank account or used to pay your Apple Card's monthly due.
