Password being modified on my iPhone

Had a bunch of trouble trying to log into a website using my saved password. Went to the password app and it oddly indicated my password had been modified today when I knew it had not. I tried to delete this password but it kept coming up even when I had changed it. After trying several time, clearing my safari, restarting my phone, etc., I was finally able to delete and change my password.

I checked some other saved passwords and oddly enough some of them had modified dates more recent than they actually were. So, am I dealing with a phone issue, malware, hacking? Something else?
