How to activate Applecare to the Ipad I bought together at Costco?

I recently purchased an IPad and Applecare for it online together at Costco. I received an email receipt for them both and the IPad has been delivered. I'm trying to set the iPad up and it keeps trying to sell me Applecare. I haven't received anything but the receipt for the Applecare. Please advise! Thanks
