Proof of Blacklist/Barring Guidelines

I have lost my Iphone 13 pro max, I wondering if is possible getting a letter proof called "Proof of Blacklist/Barring" so I would be able to submit it to my insurance company.

The document need the following details:

• Letterheaded document

• Name and address of the company issuing the document

• Customer Name

• Customer Address

• Mobile Number

• IMEI and/or Serial Number of the device

• Blacklist/Barring date
