What software did you install when you noticed this Guilan Xiao item?
There isn't much information online about either the name of the item or its developer. What few references I found to it seem to suggest it may have something to do with "sidify" which appears to be some type of music converter, but every reference to this item also seems to have just been discovered as you did. It is possible people have had it for years and never realized it until Apple provided the list of Background App permissions in macOS Ventura. Some download sites will tack on other unwanted bundles, so it is best to only download directly from the developer's official sites.
Since there doesn't seem to be any good information about this item, it does make it seem very suspicious so you should just be able to delete the file since the EtreCheck report gives you its exact location assuming it is something that was not intended to be part of any app you installed.

After deleting it, I would reboot the computer and run EtreCheck again to make sure it is gone. I don't know if deleting the file will also delete the entry in the Background App System Settings.
FYI, I notice you don't have a Time Machine backup listed in the EtreCheck report, or any other signs of an automated backup. You really do need to have frequent and regular backups of the computer and all external media (including the cloud) which contains important & unique data. There are a lot of new ways to permanently lose access to your data on these newer Macs due to all the hardware, software, and security changes that have been implemented if you don't have good backups.
