Are you using any third party password manager?

Any Firefox extensions installed— I would try disabling all and try again...

Is this a one off, or the error is repeating under the same conditions ie Firefox.

I would ignore a one off glitch.

It only happened once. And yes, I am using 1Password and have 2 or 3 Extensions running in firefox.

When the problems apear again, I will do what you said.
