Has anyone ever seen this on an iPhone???

So I’ve noticed a lot of things happening with my iPhone. Apps I didn’t download, opening up safari to websites I’ve never heard of. Being logged out of apps and websites. Passwords being changed, all kinds of stuff like that. But recently I went to grab my iPhone and this was my screen, I took a pic with a different phone to make sure I had a copy. It’s not a widget and as soon as I touched my phone the screen went to my regular screen…. Anyone have any ideas?
