We have two phones and two watches with one Apple ID. I can only set up health app on one watch and phone. The same info appears on both phones and watches. How can I set up a separate health app.

That is to be expected. Apple IDs are not and were never meant to be shared. The best option is to set up and use separate Apple ID's.
One of you'll keep the current one as your own. Other person signs out of the Apple ID on all their devices (Sign out of Store and iCloud) and chooses the option to keep content on the device, if asked, then they will create their own Apple ID and sign into it and turn on their preferred iCloud options again.
