You can try signing out of imessage on both devices, power off and on the devices, then sign back into imessage, then try sending MMS on your ipad.

To send MMS messages from iPad, in addition to enabling Text Message Forwarding on your iPhone, ensure that you have also enabled MMS Messaging.

On your iPhone:

Settings > Messages >

• Text Message Forwarding - explicitly enable your iPad from the list of available devices
• MMS Messaging - set to ON
