Disney Channel (with e.g. Raven’s Home) can be part of a pay TV provider plan (live/linear TV, like cable). Not standalone.
Disney+ (with e.g. Wish) is available standalone as subscription and app on current model streaming devices — not on legacy devices like the A1469 Apple TV (3rd generation).
The Apple TV (3rd generation) Home Screen has all the “apps” that it supports (for your country or region). Users cannot add to that, nor remove. “Apps” can be set to show or hide in Settings, but that’s it. The legacy devices do not have an on-device App Store, unlike the later models.

An A1469 which is the Apple TV 3rd generation is EXTREMELY obsolete (close to 10 years out of date) and cannot add apps. Sorry to be blunt, however It is time for you to replace your device. Current Apple TV's (Apple TV 4K 3rd generation) are only $129. The current Wi-Fi version has the App Store where you can add Disney & Disney + and many many other apps. You can find it on Apple.com
