I was customizing my app icons on shortcuts and I selected a photo and it worked but the name was different.
I did not know what to click to change the name of the app to customize. Can you tell me how to customize the app name?

Unlike App folders that can be set as you choose, the name associated with an App icon cannot be customised. The App name that you see has been selected by the App Developer - and cannot be changed by the end-user.

You cannot rename apps on iPad, but you can create a shortcut in the shortcuts app to open the app you want. You can set a custom title for the app, and even customise the app icon by clicking 'add to home screen'.
