I just noticed I have two news icons on my iPhone 15 pro max. I’m almost certain I didnt accidentally do this myself. I’m wondering if this might mean my phone has been compromised in any type of way? Or if that’s even a possible way to spy on someone? Should this even be a concern like that?

If you're seeing one on the Home Screen and one in the App Library, that's normal. There is only one.
Review your purchase history here. This will show the date that apps have been added to your phone using your Apple ID, even if they are free.
View your purchase history for the App Store and other Apple media services - Apple Support
If someone knows your Apple ID Password, they could have logged your Apple ID out and put in theres to download apps. That would explain if the apps are not in your Purchase History, but still on your iPhone
If you are unsure what the apps are, you can post a screenshot of them and I am sure someone will be able to identify them and tell you what they do to ease your concerns.
