The sleep apnea support requires one of three models of Apple Watch (seemingly including yours), and must be in one of the countries or regions with sleep apnea detection support, and the Watch must be running watchOS 11 or later (as available), and you must be 18 or older:

• Receive sleep apnea notifications on Apple Watch - Apple Support
• Models that support breathing disturbance measurements - Apple Support
• watchOS - Feature Availability - Apple

Does your configuration meet all of these requirements?

Set it up in the health app on the phone.

Did you read Mr. Hoffman’s post that has links for you to follow and read about the requirements? Does your Apple Watch and iPhone meet the necessary criteria?

if You have been diagnosed with sleep apnea you will not be allowed to use it. I bought this watch for that reason just to not be able to use it. I feel scammed.

How does Apple know you have sleep apnea?
