I understand that this has been a problem dating back to iOS 17.4, but the problems persists in ios18 and with the advent of the hearing aid mode which is dependent on the custom Transparency mode, this has become an important problem.

I have the same issue. Transparency mode shuts itself off randomly. And yes, if they are to be used as OTC hearing, such as myself, it’s a big problem. Not sure how the FDA would approve that if it’s so unreliable.
