This extension installation popup appears every time I restart. How do I get rid of it?

Try going to System Settings > General > Login Items & Extensions. See if there are any unfamiliar items enabled. If there is/are, try disabling it/them to see which will cause this prompt to not appear again.
Did you try installing something recently? ... or deleted something?
