I dialed "*#61#" on my iPhone 16 w/ AT&T cellular and got this: "setting interrogation succeeded. voice call forwarding when unanswered forwards to +125*******. enabled." Is that just my voicemail or has my phone been hacked?

That would be a question to ask of AT&T support. What the codes like this do and report can vary from carrier to carrier, so only AT&T can tell you precisely what the return may mean if anything. But no, it's highly unlikely that your iPhone has been hacked, unless it's been jailbroken in which case all sorts of bad things could happen.
