I tried to disable spellcheck and grammer check in the notes app but still I see red underlines in the notes app:
I've also checked the keyboard settings in system preferences and made sure all spelling checks were turned off.

I should have paid more attention to your original post being a Notes issue.
Open Notes.
Goto Edit > Spelling & Grammar.
Turn off (uncheck) "Check Spelling" and "Check Grammar"

The issue was the latest notes app update has math calculations autocompletion which can be disabled from
Format > Math Results > Off
Try this:
Go to the Apple menu and choose ‘System Settings’
Go to “Keyboard”
Look for “Input Sources” and click the “Edit” button*
Find the setting for “Show inline predictive text” and toggle the switch to the OFF position
Click on “Done” and then exit out of System Settings
It's already off.
Well I did that also and I've posted the screenshots which shows all of em are unchecked.
ok...you sure did.....I'm out of ideas. Hopefully someone else will chime in with the solution.
