No voicemail notifications when voicemails are received on iPhone

Are you seeing the red dot on the Phone app icon that tells you have a voicemail? If you are not, then yes, it is a carrier issue.

If you are seeing an indicator, there are other reasons you could be missing the alert sound. Check Settings>Sounds & Haptics>New Voicemail and make sure that it's set to a sound that you can here and will recognize. I find that very musical tones can get obscured by other music. If you have any age-related hearing loss, look for a tone that's low in pitch.

Do you have your phone set on silent? Are you using Focus/Do Not Disturb? Do you hear audible alerts for other things?

Regardless, I've given you all the information I can. You will need to determine if you need to contact your carrier or if you need to do more troubleshooting on the phone.
