Why does my iPhone ringtone volume increase with bluetooth?

I've seen an old thread about the topic and it still hasn't been solved, after 4 years. Come on people! This phone is too expensive to keep (almost) giving me a heart attack every time it rings. FIX THIS!!!!

For those few people who don't know what I am talking about: after connecting via Bluetooth, the iPhone increases his ringing tone to maximum value, which causes me serious stress and risk of heart failure, considering my specific medical history.

Also, WHY is the ringing volume the same as the alarm volume? WHY? WHY??????? Other way less expensive brands have figured this out. Why can't you?
