How Do I Use Keychain/Password Properly?

What are the step-by-step instructions to. use the Keychain/Password utility correctly so that it updates and keeps updated passwords properly organized?

For the past 5 years at least once a day I try to login to a site or account and find that the Keychain/Password utility has an incorrect password saved.

Obviously I'm doing something wrong but can someone explain exactly how to use Keychain utility (pre-Sequoia) and the Password App (post Sequoia) so that
