I'm delighted to say that updating to 15.4 has resolved the problem - VIPs (for me) are now functioning as they did before disappearing in 15.3.

I had the same problem. I contacted support through the chat option. They resolved the issue on their end somehow. She suggested removing Login items if it happens again. See this help article for instructions: Remove login items to resolve startup problems on your Mac - Apple Support
I’d suggest removing login items and restarting your computer first. If that doesn’t work, reach out to Apple support directly.
This is specifically my issue. VIP's work if I click an individual, but when I select the whole group it shows all the email from everywhere.
I HAVE found that if I CMD+Q and then reload Mail it works again as expected... until it doesn't and I have to restart again.
A "temporary" work around.....
I created Smart Mailboxes with the VIP's information and that seems to work in a similar manner to the "old" VIPS.
The problem recurred, so I contacted Apple Support. They determined that I needed to reinstall MacOS. This isn't a quick solution, so I'll report back later after I find time to do it.

These are not one and the same thing.

First, to clarify: after upgrade to 15.3, all of my VIPs in Apple Mail started exhibiting weird behavior. Namely, selecting any VIP would merely show me my regular inbox (no filtering was happening in the VIP view). All of my VIPs still existed, their mail still existed, but Mail was not filtering out non-VIP email when selecting any of my VIP views.
Here are the steps that I took to try to resolve (to no avail):
I restarted Mail - nothing
I rebuilt my mail boxes - nothing
Restarted Mail after rebuilding mail boxes - nothing
Removed all VIPs, restarted Mail, added my VIPs back, restarted - nothing
Rebooted the Mac - nothing
It's as if all of my VIPs have been relegated to the status of Commoner now, they are no longer special - perhaps a coup? Power to the People?! In all seriousness, I would like to have my VIPs back.
I have had the same problem as everyone else here. Spent about six hours with tech-support with no solution. Except to downgrade to Sonoma, which I will do until Sequoia is fixed.
I have Gmail in addition to Mac Mail, and suspect this may be an issue of MAC not playing well with Google
I just finished a chat followed by a phone call from Apple Support. They have assured me that the engineers are aware of the issue(s) and are working on fixing. When the next update comes out, the advise to proceed with it straight away.

Good to know. I haven’t tried to reinstall the OS because I’ve found using a smart mailbox works just as well as the original VIP mailbox.
My VIP list is a mess as well. After the update, there was no difference clicking on a VIP then than the entire inbox. I called support and was instructed to "reset" each email address and restart Mail. this fixed nothing and may have made it worse. Is there a way to revert to the prior Sequoia update before adding the AI to the email??
Something else I noticed...even searching a specific address in Apple Mail will give me the most recent communication and then nothing after July of last year. These are individuals I communicate with weekly or daily in some cases. The e-mails are still in the inbox but I have to scroll down within the inbox to find them...WTH??
My VIP folder is a useless mess since 15.3. It's my VIPs plus all of my sent emails and any email they decide to randomly add to it, including junk. I can temporarily fix it by quitting Mail, but as soon as I view any other folder, when I go back to VIP it's the same mess again.
I have the same problem after updating to 15.3.1 (24D70). One difference I notice from the other replies, my VIPs are sorted, but only for a few seconds after opening mail app. Then they revert back to the unfiltered general inbox after those few seconds.
Apple please help us!
