Spotify will not download to my child’s watch and only says “requires iPhone”. We have linked through our family account to my iPhone, we have Spotify premium and we also went through the process of deleting the Spotify app and reinstalling. We also unpaired the watch, deleted the watch pairing app and reinstalled everything. This did not help. We had no issues with my son’s watch pairing Spotify to his phone.

Hi. Is your child's phone not an iPhone? If it were, are you not able to pair their watch with their phone and then the Spotify companion app should work. You'd obviously still need to share your premium subscription to their Spotify account.
If the child's phone is not an iPhone and you're restricted to pairing child's Apple Watch with your iPhone, regrettably that's not currently a supported setup.
Without an iPhone that is paired directly to the watch, Spotify seems not to work in the setup for Apple Watch for Your Kids.

Unless the developer offers this option, there is nothing Apple can do about it.
Leaving feedback via the mentioned Idea link directly may help.
Setting up watch "for a family member" is mainly for kids that are too young to have their own phone.
Hence some limitations.
