Why can't I download Spotify on a Family managed Apple Watch?

I want to connect my son's apple watch to spotify, so that he can listen on his watch. He does not have a phone, and the watch is managed through my iphone. We have Spotify Premium, but when I try to download the app on the watch, it says that I have to do it on my phone, and there is no way to download apps on my phone since it's a family watch, and not my watch.

Certain apps on the Apple Watch require an iPhone to download, because this ensures that both devices are using the same Apple ID and apps and services can sync and communicate between the two devices. Spotify is one of those apps, so unfortunately, you cannot download Spotify for your son’s Apple Watch.

I ran into this issue a while back myself.

It's called "family watches". It's for using an Apple watch for a child- someone that does not also have an iphone. I'm sure there are other threads where you could read about the details. I have an iphone, that uses my apple ID, the watch uses my son's apple ID, and I manage the watch through the app on my iphone. The difference, from what I can tell online, is that typically you can manage apps on your iphone for the watch, and in this instance you cannot.

… my son's apple watch …
… the watch is managed through my iPhone …
… since it's a family watch, and not my watch …

In Apple’s system, any device is registered to someone’s Apple ID. Any Apple Watch will be on the same Apple ID as some iPhone, and be paired to that iPhone. There is no concept of family items. All is individual.
