Am no expert in this area, yet your post to the community deserves some response.
You may want to check out Apple's "Shopping help" payment info. for tips. Perhaps a detail was overlooked.
Eg: says you need to be signed in. Also says there is a maximum of 8 gift cards for a purchase ... info on using more than one payment method for a purchase is there too, and more.

As authorisation for a payment has failed ...
- it may have something to do with your financial institution not verifying you are you... (bank / card-provider etc), even though you want to use your Apple Account balance for the purchase.
- it could mean that your address listed in your payment info. does not match the delivery address you entered for the purchase
