My wife has my apple id on her phone!
How is it possible that my apple id is on my wife’s phone?
She went to download an app and it showed her my apple id and asked her for my password to download the app.

Only possible if someone logged in to your Apple ID on her iPhone for iTunes & App Store purchases at any point , and never logged out.
On her phone, go to Settings ➜ Apple Account ➜ Media & Purchases and verify which Apple account is logged in there. If it's yours, then log out and log back in with her account.
