For the last few days, the microphone has been constantly on, and the orange dot is lit. I tried turning off microphone access for all apps, but the orange dot is still lit. The information line says "Privacy, Accessibility." What does this mean? And how do I turn it off?

Did you turn on Vocal Shortcuts in Settings > Accessibility? If so, the microphone stays open so that it can detect the key words for the shortcuts.
