I have the Apple Watch SE 2nd generation and it never charges up to 100%. It did it at first, but won’t now even on another charger. Also, it asks for my passcode ALL the time and it is aggravating!

Did you check the settings for Optimized Charging?
You can disable this feature and charge your watch to 100% each time you put it on the charger.

As for the passcode, did you make sure that Wrist Detection is enabled and that there are no tattoos underneath the sensors?
