That is lens flare and happens on all cameras, even the most expensive ones. It is the light from a bright source reflecting off the inside of the lens. Having a protective cover over the lens is irrelevant.

Do you have a protective cover over your lenses?
That green dot on your screen I believe you are referring to indicates that an app is using your camera, which in this case is the Camera app. But it shouldn't move and should be fixed in the Dynamic Island.
