That is EXACTLY it. Attention Required is a security feature; it verifies that your eyes are open and it can see the whites of your eyes.

Even if it happened after updating, it is still a hardware problem that may have occurred coincidentally with updating.

This exactly. It’s not a broken camera. It’s all over Reddit and other sites. Both my iPhone and my iPad are experiencing this. I can’t believe thousands of FaceID cameras broke this month.
