My iPhone 15 Pro Max, with the latest updates, is wrongly allowing access to my phone, while I'm asleep and/or have my eyes closed. I have the correct settings enabled but it is scary to know that someone can access my phone even while I'm asleep

Not quite sure how you know it has opened if your eyes are closed, the second you open them it will unlock. Or how you access it while asleep.
Anyhow go to Settings → Face ID & Passcode, turn on Require Attention for Face ID.
