Re: "Screen Unknown Callers - Ask Reason for Calling
.... this option does not appear under Settings > Phone > Screen Unknown Callers".
✽ Wondering if you have you tried checking whether Call Screening is supported by your carrier.
✽ You might want t check whether all unknown callers are already blocked

✽ If not already turned on: Wondering if turning on call idetification first may help ?
Apple support's online article: Screen and block calls on iPhone - Apple Support
Eg: says:
"Turn on call identification
iPhone can help identify incoming calls using information from Apple Business Connect, supported carriers, and supported call identification apps.
- Go to the Settings app
on your iPhone. - Tap Apps, tap Phone, then tap Call Blocking & Identification.
- Turn on one or more of the following:
- Business Call Identification: Caller information is identified by Apple Business Connect or your carrier.
- Call Identification apps: Caller information is identified by supported caller ID apps.

Re: "The “Screen Unknown Callers” button appears, but there are no options like “Ask Reason for Calling”.
A possibility: If you've previously chosen an option like "Never" or "Silence" (perhaps by accident) may be that needs to be turned off first.
Above linked article also says:
- "Go to the Settings app
on your iPhone. - Tap Apps, then tap Phone.
- Choose one of the following:
- Never: Call Screening is turned off and your iPhone will ring when you receive a call from an unsaved number.
- Ask Reason for Calling: Calls from unknown numbers will be asked for more information before iPhone rings.
- Silence: Calls from unsaved numbers will be silenced, sent to voicemail, and appear in the Recents list.
Note: When answering calls, Siri will speak in your iPhone’s default language. See Change the language and region on iPhone."
