Whenever I try to connect my personal hotspot from my iPhone to mac, most of the time it says “FAILED TO ENABLE PERSONAL HOTSPOT” , even if it’s on and near the device.

Follow steps here If Personal Hotspot is not working on your iPhone or iPad (Wi-Fi + Cellular) - Apple Support
IF you still have issue contact your phone carrier.
