From what I was able to glean from a search of Apple Support and the Internet in general, sleep scores are calculated using respiration rate, heart rate and movement. The Apple article below offers much more detail.

I ran into the same issue while looking for an answer specifically from Apple. Other than the article I linked, I was only able to find information on sleep trackers, in general. You could try contacting Apple Support for help with this question.

Do you think it could measure sleep stages without heart rate?
Try turning heart sensor off in iPhone, Watch app, My Watch tab, Privacy and try sleep analysis that night,
