I'll ask the question a different way: If you slowly point the camera toward the left, or downward, does the spot stay in the same place on the screen?

OK. Be sure to try a Forced Re-start:

This is different from a simple Power Off. I had to continue holding the button down 5 or 10 seconds past the "slide to power off" message, and then I released the button only after the Apple Logo appeared indicating a re-start.
Have any parts of the iPhone been replaced recently?
You should get the iPhone checked by visiting an Authorised Apple Service Provider or Apple Store next to your place.
To me this looks like a particle inside the iPhone /camera lens.

Probably only one. I would expect over $150, maybe more. Independent shops can be cheaper (and maybe less dependable) than Apple. Estimates are free. Sometimes the service provider can help.
Does this smudge always occur in the same part of the picture and others, even when you zoom in or out? Does it only appear with the .5 lens?
There are some other spots-- do they persist at the same places in other pictures?

Only on the 0.5lens - the one on the mouse.
The others you point out are a USB cable (right side of the mouse )
Thank you for pointing out the ones on the left-hand side - didn't notice how light they are.
I did a forced reset, and nothing. I added a video where you can see it goes away once you change lenses.
I made this video, and you can see the change of lenses and when I shake the phone, how clear the spot is.
