I have similar issue.
When somebody calls me, although I have his number saved in iPhone Contacts, Contact name is not displyed but only number.

Strange thing is that when I check Recent calls (log) later, contact name is properly displayed.

Number displayed when receiving the call is completely the same as one saved in my contacts ('+', spaces, everything the same).
Also I dont have duplicate contacts with the same number.

iPhone is new and just activated.

i was having the same issues with my iPhone 4S. I reseted all settings and started the phone like new. I set up the phone as a new phone not an icloud or itunes back up. Once it was set up, I then synced to icloud and everything was good. It thought I was in the netherlands but I am in the USA. This fixed it fo me. Hope this helps.

I've updated my iPhone 4 to iOS5 and I'm facing the same problem. 95% of my contacts saved with the international prefix (+ xxx xxxxxxx) since the update not all incoming calls but most of them show the number without the name and prefix although it's stored in my contact. when I go to the log and dial that number it doesn't show the contact name ... I have to go to the contact info. and edit it by erasing the prefix, then it will show in the log the name of the caller. I Have tried changing regions in Settings but haven't managed to solve the issue. what's clear to me that I need to clear all prefixes stored to only original number which is something i don't want to do, Any help will be greatly appreciated.

p.s. I'm using the phone in Kuwait

I backedup to my computer with my iPhone4 and then puchased the 4S yesteday. When I restored, the numbers/call history/ everything appeared accept the contacts. Under contacts, nobody is listed, but my call history and texts are all there with numbers (no names). I backed up my 4 not using the latest software and it appears iTunes automatically updated to 5.0 when I plugged in my 4S, so I am wondering if that is the issue. Any advice is appreciated on how I can get my contact names back.

same problem here as shahy's.

in Egypt on the 6th of october, all mobile numbers have changed from 10 digits to 11 digits (operator code included).

now my phone will not display contact names properly. This issuing has impacted:

-all my previous messages, threads appear as numbers and no contact names (I am guessing since an extra digit is added)

-now when i send a message to a number and receive their reply, their replies open in a new thread. so basically now i have two threads for the same contact, one for outgoing messages to this contact and one for incoming messages from the same contact.

-all whatsapp contacts have disappeared, even those who updated their numbers on whatsapp itself with the 11 digits.

-favourites list now is a mix of numbers and names.

Please, any help is highly appreciated. My iPhone contacts are such a mess that i do not know which number belongs to which contact and who sent this sms.

Info:

-iPhone 3G IOS 4.2.1.

-Windows 7 Home Premium.

-Latest version of iTunes (10.4 or 5 can't remember, updated a couple of days ago).

-Old number format: 01XYYYYYYY (X depends on the operator. ex: 1,4,7 for operator 1 / 2,3,6 for operator 2 / etc...).

-New number format: 01ZXYYYYYYY (now Z is the operator code. ex: 1 for operator 1 / 2 for operator 2 / etc...).

I figured out using AT&T mobile transfer App. You can transfer Apps, Contacts and Photo's between phones! I gave my old phone to my brother living in a different state, so we both downloaded the App and it worked perfectly for restoring my contacts, The pictures associated with each are messed up, but at least I have my contacts back! If you are an AT&T customer, I highly recommend using this app!
