I have similar issue.
When somebody calls me, although I have his number saved in iPhone Contacts, Contact name is not displyed but only number.

Strange thing is that when I check Recent calls (log) later, contact name is properly displayed.

Number displayed when receiving the call is completely the same as one saved in my contacts ('+', spaces, everything the same).
Also I dont have duplicate contacts with the same number.

iPhone is new and just activated.

I tried the *228 fix, and that did nothing. I have a few observations, and i wonder if anyone can confirm these. First of all, it seems to be an issue plaguing only CDMA iPhones (Sprint and Verizon people). Second, i wonder if it has to do with the location of the Contacts in the cloud. Unfortunately, my Mac is still running Snow Leopard and isn't powerful enough to upgrade to Lion, so i am unable to use iCloud. I got my 4S after having had an android phone, so my contacts were all in Google Contacts. I reverse synced google contacts back to my phone, and i periodically have this issue. OH, i also restored to an earlier backup... it stopped for a couple of weeks, then started up again. I can't seem to figure out the pattern, and i have no idea what is triggering it. It's starting to get very frustrating!! I am beginning to wonder if the only solution is to print out a list of all of my contacts, delete all contacts from my phone, Mac address book, AND Google, and start from scratch.

Yes, only CDMA phones - in the US. But this problem seems to be fairly common outside the US.

I am guessing for us outside the US the problem exists if we're using unlocked iPhones on operators which have no agreements with Apple.

A UK user (michaelfromstapleford on page 4 of this thread) mentioned that the problem could be fixed by resetting all settings on the phone. I tried this but on my phone it only worked for the first incoming call!

Okay, so my problem has been fixed. The *228 solution really does work for Verizon customers in the US. Apparently, when I tried it myself, I didn't allow sufficient time for Verizon to reset my account. The genius at the apple store (UWS NYC) said he had seen this often, but only with Verizon phones, and they only show the Netherlands; never any other country. He performed the *228 Verizon reset a couple of weeks ago, and I've had no problems since.

Phone: iPhone 4

Version: 5.0.1 (9A405)

Modem Firmware: 04.11.08

Factory Unlocked: Yes

Jailbreak: No

I am also facing the same issue like the rest of us here. I am using phone in Maldives. The only way my phone would show a contact's name is when I insert the phone number without the international code. Like for example, if I insert +9601234567(even tried 009601234567), it wouldn't show the name, but if I insert 1234567, it would display the name in the incoming call. I am facing no issues with the name display in text messages (whether with or without international code). Now, I am hanging in between whether to go ahead and remove international code from all of my contacts; render my contacts completely useless when I am outside the country & then enjoy the primitive feature which is even available in the basic & cheapest phone available in the market or leave the contacts unchanged and memorize all the names and numbers by heart.

A friend of mine who is an Android user does not face any problem regardless of how he inserts the number of a contact. It would always display the name of the contact. Will Apple going to do something about it? Update? Patch? Anything?

Did you try resetting your settings? That seems to work for some. Not for me though; only the first incoming call shows with name, the ones following show again as numbers. I am waiting for iOS 5.1 - hoping it will fix this problem. But it seems doubtful since Apple hasn't acknowledged this bug. Maybe they don't even know?! (Feedback is diverted to /dev/null?)
