OK, I was having this issue on my iOS5 (5.0.1) iPhone 4 on AT&T. For me, it was that SMS/iMessage thread from my wife was showing up with number and not name, even though I clearly had her in my contacts in the same exact number format. It wasn't doign this with all contacts. Others mostly showed with name properly showing up. it was only hers that was not showing.
All I did was go to task manager and kill the Phone, Contacts, Messages, and Mail apps. Then I went back into Messages and noticed that now it shows the name properly for hers and all others.
Hope this simple fix helps for some of you!
Unfortunately, when I dial this, it tells me it cannot connect and to call using a land line. I think this phone is cursed. The associate at Verizon had a hard time just initially activating the phone and my bad luck with it continues. Guess if I want this fixed before Apple comes out with an update, I'll have to take a trip to the Verizon store. *sigh*
i have the same problem with my iphone4 ios5 and i have tried your solution but every time i dial for *228 it say that this is a wrong phone number! maybe this number is not working in my country region (kuwait)
any other suggestions?

This totally worked. I dont know about the task manager but I called the number, programed it, and rebooted the phone and all is now working!!!!!

Can confirm that this solution does indeed work.
Siri stopped working for me a about a day ago so I performed a settings reset to fix it. Afterwards I had this issue with the contacts. Fixed by activating phone via *228 then a task kill and phone reboot. Verizon, of course.
I'm living in UK and fixed the problem by restoring all settings. Settings > General > Reset > Reset All Settings. This resets all your settings but keeps all data including contacts, media and emails.
This bug had been annoying me for ages. Really glad it's now sorted.

Husband managed to fix the problem using this.
I recently upgraded from iPhone4 to 4S AND simultanously switched from AT&T to Verizon. I had the exact same [combined] problem of Contacts not translating to Caller ID, Messages, Recent Calls, Favorites, etc AND the phone thought all of my US contacts were in the Netherlands.
The *228 fix worked perfectly in correcting this issue. Thanks!
I'm with Sprint. Mine just happened today. The issue is only with one contact that happens to be the only I call and message frequently. I did all the resetting, task manager clearing, and obviously dialing *228 wont work. Anyone else with Sprint having the same issue?
I have a Sprint Unlocked phone (I am using it in Costa Rica) and the Iphone started erasing my contacts and then they didnt show up neither on recent calls nor on messages, for one contact I added the country code +506 before the number, that WORKED but I dont have the TIME to change all my contacts one by one... I did the reset Settings, and it worked...
The *228 fix worked for me. I unlocked my verizon iPhone 4s the other day and put in a foreign sim card. When I when back to the Verizon US sim card I'm assuming this caused the problem. Thanks.
