is it safe to download the mackeeper?

Does one not need to have something to keep one's Mac clean and healthy or is it simply not required?

I would say to use the tools included on your mac.

But if you need a 3rd party i would say use CleanMyMac as that is not a scam

Yes. That is just as true now as it was six years ago. It is still a just as worthless of a scam now as it was then.

I've had issues in the past when using the clean junk feature, removed something important and I had to do a system restore :/ stopped using it about 5 years ago

No no no and no Mac safe keeper will only give you Popups ect if you downloaded it dont open it just delete it i got it my accident and i deleted it and i didnt get pop ups if you didnt open it
