I believe that I have spy software on my phone. From what I have seen advertised online phone calls, numbers, SMS/MMS and email can all be monitored. The iPhone 4 is one of the smart phones listed on these various sites. How can I find out if this software is indeed on my phone and how can I get rid of it? Any help would be greatly appreciated.

Why do you believe that you have spy software on your phone?
Erase means erase, your phone will be erased and the stock firmware image will be loaded. (just like when you took the phone out of the box)
Not exactly... doing that on a hacked or jailbroken phone can result in a phone that can't activate, or possibly even can't boot.
ic.... however stupidly i forget my restriction passcord, cannot reset......
