Interesting that there has been a Mac iTunes update since this problem first appeared. That update did not, however, include a fix for this unwanted login window. The solution offered by "jh366" appears to have no Mac equivalent? I am still getting the sign in mesage twice. I am starting to regard pressing ESC twice after launching iTunes as standard procedure 😟

I am on Safari Version 5.1.2 (7534.52.7).

Mac OS 10.7.2

I have found that this issue occurs after I select Reset Safari... from the Safari menu.

I have completed all the other previously suggested items, but found that it does not matter. Once I select Reset Safari... it resets the iTunes Store password for Automatic Downloads.

Not sure why these are linked. This is very annoying.

I discovered that signing in to the iTunes store in iTunes creates a cookie on the Mac. If you have an extension in Safari that deletes cookies on exit, you may be deleting the Apple iTunes cookie, and it will make you authenticate on iTunes startup. For me, I signed out of the store in iTunes, signed back in and then made sure I had the cookies from Apple exlcuded from auto deletion upon Safari exit.

At least on the Mac side, artimon is 100% correct. Now I know why iTunes has been acting weird the last few weeks! Thanks for that.

I recommend TechSTL check his browser settings. If he's using Safari, then that might be the cause. I imagine iTunes on windows might rely on an internet explorer cookie, since Safari is not guaranteed to be installed.
