Not sure if you ended up solving your problem or not, but for others who are looking for a solution, I had the same problem--and figured out an easy, uncomplicated way to fix it! Earlier today, I downloaded an application to my computer (it was a screen recording one). After launching it and using it, whenever I tried to raise the volume, my sound icon showed up but it had a little crossed out circle under it. At the top of my screen, where people can also adjust their volume, the icon was greyed out. I solved this problem by going to System Preferences>Sound. Then, there should be three sections: sound effects, input, and output. Under each tab, there should be a drop down menu that decides where sound comes out. On the sound effects tab, it says "Play sound effects through". Next to it should be the drop down menu. Click on it, and make sure it's selected as "Internal Speakers." Then go to the input and output tabs, and make sure it's all selected on Internal speakers. This worked for me! You may not be in the same situation as I was, but hopefully this helps at least one person! 🙂

This worked for me as well! So much easier than the other fixes. Mine just greyed out for no reason.
I did not have to reset permissions. Both my user and group were "_coreaudiod" for that directory and the plist file it contained. Just ran the last two commands, clear cache, and restart daemon to fix the problem. Resetting PRAM doesn't sound like good advice. Thanks.

I Have the same problem after using headphone jack. After trying all the suggestions rebooting, Pram, disk repair etc all to no avail I realised it is mechanical problem with the jack. Fixed by moving headphone plug around in the jack until it sorted out . There must be a switch or something that gets stuck, I think.

I went to Apple Button > System Pref > Sound > Output
Then I changed the output to the built-in speakers
Then I plugged in headphones
Changed output to headphones
That fixed the problem for me. Hope that helps! 🙂

As a slight variation on your recommendation ...
I have the "Pro Tools First" application installed.
In System Preferences > Sound > Output, "Pro Tools Aggregate I/O" was selected (though I have not used the Pro Tools First application in a quite a while, but I have used Logic Pro with my MOTU 8 Pre as sound output recently which is what I believe caused the system setting change).
Choosing "Headphone Port" (my headphones were plugged into the audio jack) un-greyed out the Sound icon.

Literally, I read this whole thread. One person said, "Blow into the headphone jack." Done and done. Its not a software issue. Its a hardware issue. Clear the lint out of the jack.
