From a previous answer to this question:
Buy one from the iTunes Store and install it, or create one yourself and sync from iTunes.

There's lots of other information on this out there, Google: "add ringtone to iphone 5"
Here's the step-by-step screen capture for your easy reference! 😉
Step 1: This is the home page (summary page) you'll see once you connect your iPhone to your computer.
Now, select the "Done" button at the top right hand corner.
Step 2: Now, you'll see the music list in your iTunes.
Click on the drop down tab showing "Music" currently.
Step 3: ...and select "Tones".
Step 4: After you're at your "Tones" page, open up your folder where you save your ringtone.
Make sure you've already converted it into .m4r format.
Select the ringtone you want to use in your folder, then drag and drop into your "Tones" page.
Step 5: You may double click on the clip to listen (to confirm) before proceeding.
Next, select (highlight) the clip, and drag towards your right. Your device panel will automatically shows up on the right.
You'll need to drag the clip till it highlights your device's name, and drop it (release your mouse) = copying into your iPhone.
Step 6: Now, click on the iPhone button at the top.
Step 7: It will bring you to your music list on your iPhone.
Step 8: Now, select "Tones" on your left panel, and you'll see that the ringtone clip is already in your iPhone's "Tones" list.
Step 9: Now, click on the "Sync" button at the bottom right and you are done!

then a tab will appear click on browse and pick up your song and trim that wherever from you want and then click on generate after a second your tone will appear in ringtones

Just used the steps in this thread and got things working fine. It seems counter intuitive (creating/converting an AIFF to an ACC, then dragging it to the Desktop and deleting it from iTunes and then editing the suffix and then...) Just do it. It works. Part of the problem is that converting an AIFF to (supposedly) an ACC actually ends up with the file having ".m4a". "I thought I asked to convert it to "ACC"?! Well, you did, but its generic type/suffix is still m4a. ;-)
What have you tried?
well i have changed the track length to 30 seconds, then i created an AAC version, and changed that to an .m4r file. i then deleted the first one off itunes and dragged this new one in.
but it would not go onto the Tones tab in itunes, although it recognises it as a ringtone. and i cannot find it on my phone :/
When I update my iphone, my ringtone disappear. Now no matter what I do using all the methods that was recommended, I could not sync back my tones for my iphone ringing tone. Can someone please advise? many thanks.
Nahla, I am having the same trouble, my ringtone will not show in Tones, stays in Music and does not show in the tones for sync to the I phone. Did you get a resolution to this? Let me know. Or if anyone else knows, please help. I am using a Mac and the version of ITunes is 11.0.2. Thanks a lot!
Hi Maria / Hi Everyone,
First you have to login iTunes before your sync RingTones. Otherwise you won't be able to add tones. Follow these steps.
1. Open iTunes. Click on the small icon from the top left corner (which may contain iTunes settings)
2. From the drop down menu, Select 'iTunes Store". Select "Sign In". Sign In please.
3. Authorize your computer now. It's in the same list/menu (You may ask to enter Apple ID & Password)
4. Now go to iTunes "Tones" section. Put a tick on "Sync Tones" and select "Selected tones"
5. Again go back to the previous icon, click on it and select "Add files to Library" (This option is unavailabe without Signing In)
6. All right, now a window will pop up. Find your tones & select (you must have backups in your PC). Click Add/Open to finish adding tones. Selected tones will be added and appear under the tones section.
7. Now select tones which you want to add (Put a tick)
8. Click "Sync" button from the Bottom. Now go to the iTunes "Summary" page and again click "Sync" button from the bottom.
You are DONE. Check your phone's, Settings > Sounds > Ringtone

I followed the advice in Jim Hdk post link:
http:/How to sync ringtones to your iPhone - https://discussions.apple.com/docs/DOC-3792
the ringtone i had created would show in the itune Tones library on my Mac but not transfer to the iphone Tones library.
This was resolved by unticking the m4r track in my Mac itunes Music library; then synching again as per Jim's post.
Sounds strange but it worked for me
I hope this helps
Andy
Hi Langtoo,
Thanks for your help. I had given up, followed all the advice and it still didn't work. I've just had my iGenius (14 year old son) on it. We deleted it from iTunes music and it popped into the Tones file.....very weird!!
Cheers,
Crazy Rat Lady.
I had a similar problem. The tone showed in itunes app section but refused to go into the tones section and it wasn't syncing to my phone (iphone 4s). After much frustration I got fed up and simply made a folder on my desktop called "TONES" then added the ringtone into that folder. Then, I pulled up iTunes and clicked on the tones section. Went back to the folder on my desktop and simply moved the ringtone with my mouse into the tones section on iTunes. It went right in! Afterwards, I went to the top of itunes and clicked on File, devices and, 'sync device.' The ringtone went to my phone.
I give up. I've done everything EXACTLY as explained in multiple tutorials, but when I get to the end, the new tone shows up in iTunes' "tones", but when I click on the phone tab and then tones, there are no tones visible under the "sync tones" section. Hopefully I can find a different way, because this one ain't working.
THIS way has NOT worked for me! Im getting very mad!!! LOL! Ive purchased a number of songs through the ringtone section on my IPhone 5 & I still can NOT get them onto my phone! Ive tried a number of times all the ways above & nothing is working! Can someone please HELP ME???!!!
The annoying thing is having to go on your pc to do this!
I have purchased ringtones through my iPhone and I can't set it as a ringtone! It's ridiculous! It should be a lot easier to do than it is..bad show apple
