We just had the same problem with a new iPhone5. It worked for about a week on our home network and then stopped. It would work on other networks though. I jumped through all of the same hoops as was listed here with no luck. We are on Verizon and have a Verizon Jetpack as our router so I talked to them. In the WI-FI settings on the router there is an ACL tab that listed MACs that were denied access. For whatever reason, the new iPhone's MAC was listed there. I cleared the entry and the phone was able to connect afterwards. Other routers may have a similar list.

Nice find (ACL in router), but it did not address my issue. I hope it helps others.

My issue is fixed however. My iPhone 5 was replaced under warrenty, and I have not seen the problem since. I can now connect to any router I want and do not seem to have a problem. This, unfortunately, helps nobody. Apple, Apple, help?!

Edit Reason: Clarification -- My iPhone 5 was replaced under warranty for an unrelated (messed up lightning port) issue. Apple did not warranty replace my iPhone 5 because of this WiFi issue.

Edit Reason 2: Spelling & grammatical corrections.
