How do I get the table of contents to show in iBooks Author

Hi Folks,

I've been adding chapters, sections, and pages using the "Add Pages" function of iBooks Author. Each chapter gets it's own table of contents, but I don't get a full table of contents at the beginning of the book. How do I get a full table of contents?
